Contest Terms

Only one entry per person.

1. Contestant must between the ages of 18 and 65.
2. Only complete entry forms will be considered.
3. In order to assure that all finalists will be candidates for LASIK or a related laser vision correction procedure, contestants must agree to an examination if contacted for one. A dilated eye exam will be required.
4. Khanna Institute cannot guarantee the final result of laser vision correction. All reasonable efforts will be made to assure an excellent final result.
5. Khanna Institute respects the privacy of its patients and will not share personal information of the contest entrants with third parties.
6. Employees of Khanna Institute and their immediate family members are not eligible to participate.
7. The Free LASIK giveaway has no cash value.
8. The Free LASIK giveaway is non-transferable.
9. All submissions must be received by December 31, 2008.
