Terms and Conditions of LA Galaxy Promotion Special

  1. Surgery Price is $6000
  2. 7 days from game to schedule Consultation
  3. 30 days to have or schedule SRX
  4. ALL tickets are available for the promotion – including: comp, season, ticketmaster, stubhub….as long as it is a ticket is for a 2008 LA Galaxy home game.
  5. Valid only for LASIK surgery
  6. Must be a candidate for LASIK surgery
  7. Has no cash value
  8. Cannot be combined with any other offer
  9. Contestant must between the ages of 18 and 65
  10. Only complete entry forms will be considered
  11. In order to assure that all finalists will be candidates for LASIK or a related laser vision correction procedure, contestants must agree to an examination if contacted for one. A dilated eye exam will be required at a cost of $150.
  12. Khanna Institute cannot guarantee the final result of laser vision correction. All reasonable efforts will be made to assure an excellent final result.
  13. Khanna Institute respects the privacy of its patients and will not share personal information of the contest entrants with third parties.
  14. LA Galaxy not responsible for any medical procedures arising out of redemption of tickets/promotion.
