CATS
Today many of my patients were cat lovers. But one amongst them from Los Angeles, had cat hair all over her clothes. Not a big, except this could cause inflammation following LASIK. So grooming of cats, shampoo-shower a good idea. Fresh clothes prior to surgery is essential.

Claustrophobia
I wanted to help this man. So I used instant psychotherapy, with meditation to relax him. We had to keep moving him in and out from under the laser.
What was really rewarding that though it took longer, we were able to do a great job with accurate flaps and well centered laser ablation.
We treated his claustrophobia and correct his vision at the same time.

Glare and fatigue
Did you know that glare can be more tiring than actually running a 100 m dash?
What is glare? The reflected light from screens, shiny objects, ice and water.
Usually the amount of exposure is not much or constant. Dark and polarized glasses cut off the glare and make vision better. A new and common nuisance is the constant glare from the computer screens. What can be done? Install antiglare screen or better still switch to LCD monitors. Unlike Cathode ray tube (CRT)monitors, the radiation and glare is less. Therefore they are easy on the eye.

SUPERLASIK
Things are moving forward and new and advanced procedures are being developed for those who originally could not receive the standard Lasik procedure. Dr. Khanna has introduced a brand new procedure called “Super Lasik” a new, safer and more stable procedure that has now been created for those who have thin or abnormal corneas. What is great about this new procedure is the effects are more precise and long term. This procedure is a culmination of the merger of benefits of both PRK and LASIK. To accomplish this cutting edge surgery, Dr Khanna uses a new FDA approved instrument called a Epikeratome. This instrument raises an approximately 50 micron flap (thinner than even an Intralase). Laser surface ablation using the latest generation of LASERS is than done. Like in LASIK the flap is riposted back. A bandage lens is than placed over the eye.
Anyone over the age of eighteen can benefit from corrective vision surgery. The investment pays for itself. What’s really exciting is that Superlasik can even be done on patients who have thin corneas and were not candidates for LASIK. Its also safer since minimal trauma is done to the cornea.

Am I too young?
In the procedure of LASIK suction in the range of 60-80 mm hg is created using a suction pump. The intention is to try to flatten the cornea which is normally curved. At the peak of the suction a blade or LASER is used to shape a flap. The laser achieves this by multiple bursts of picosecond laser energy applied ata constant predetermined depth.
The blade may be metal or diamond.
Microkertomes may be Rotational or Transitional
CUSTOMAIZATION OF THE LASIK FLAP
1. Ring Size The larger ring size leads to a larger flap. It also leads to thinner flaps
2. The stop Controls where the blade stops its motion thus determining the size of the hinge
3. The speed of the oscillating blade. The faster it moves the thinner the flap will be
The FDA APPROVED age for wavefront LASIK is 21 years. If the refraction is stable 18 year old can have LASIK. Some surgeons do LASIK in selected eyes of younger patients.

Invention of LASIK
You may have read in many websites that your local LASIk doctor invented Lasik Surgery. This claim may be equated to a former presidential candidate claiming to have invented the internet. Lets give credit where its due. To the Geek, I mean the Greek- Ionis Pallikaris.(In greek it probably means I am going to be rich)
Well you know the jocks. They could not let the dorks win so they came up with LASEK,than epi Lasik(soon there will be underlasik, than behind LASIK and than wonderLASIK).

In the begining

In the beginning there was no Lasik. And there were very few people with nearsightedness (myopia for Latin enthusiasts). Why? The answer is simple to believers of Darwin. The dinosaurs ate the people who could not see them. Ouch. May have been good to be farsighted (hmm..). That is why the majority of earths population is such.(I have no idea about the refractive status of the aliens amongst us. And no person survived to reach the age of presbyopia. That is why the refractive surgery business was not very popular in those days.
A myopic fellow by the name of Kurneah was socked by a friendly dino named Retina. He yelped in pain aei, iee, iea,eye! To overcome pain he probably applied an ice pack(he forgot to apply for a patent). Lo and behold he could suddenly see clearer. Those were the humble origins of Luxotica (and better 1 0r 2). Other methods of treatment were –activating the third eye, looking at the sun(where was FDA when needed?)
Some say LASIK surgeons invented the eyes. Otherwise why the need for friends of starfish, plants, and hydra to suddenly see anemone. Well I say definitely this was not the case. If we had invented eyes we would all have had 32 like the livelihood of the dentists, (No flossing-just polishing them all would be enough)

Do you think all LASIK are the same?
Just like all cars are not the same, even though each one ususally takes us to our destination. LASIK quality may vary depending on the surgeons experience, LASER used.
It has become much safer since the early days of LASIK in the nineties.
The tracker can follow all micro movements of the eyes.
Experienced surgeons know the limitations of LASIK
Current 4th generation LASERS are more precise. Utilizing knowledge of maths and physics the shape of cornea is maintained similar to that before surgery. This reduces aberrations, which was another cause for visual dissatisfaction. Early lasers were whole beam lasers like carpet-bombing, Newer ones are flying spot or variable spot, yielding precise placement only where needed
The diagnostic Wavefront devices can map visual errors more accurately and even show the surgeon what the patient is seeing.
The flap designing instruments are more refined and tested. They are more friendly to the eyes.
Sterliazation techniques have improved.
Blend zones (a way of applying the computer controlled laser) and larger zones have decreased the incidence of glare and haloes.
More than 5 million procedures have been performed. The cumulative data has yielded learning points which would have taken a surgeon a lifetime to accumulate. Dedicated LASIK surgeons have access to this data, and can use the wisdom of their peers. As the computer chips have advanced they have increased the speed and response rate of the lasers. The surgeon has power to override all these gizmos. Hence an attentive surgeon with quick reflexes is still the most important.
If not candidate for LASIK, Advanced surface ablation or epilasik an option.
Blended visio for better distance and near reading.
Newly discovered drops reduce pain and increase tear production. Modern materials help preserves tears and keep the eyes moist
